| | Employee Benefit Package Our employee benefit package for full-time personnel (40 hours per week) includes: - Medical Benefits are provided for the employee at no cost. Currently the plan is provided through Healthnet. Employees wishing additional medical coverage for their spouse or family members may do so at 50% of the incremental cost of such plans. Coverage begins on the first of the month following thirty days of employment.
- Life Insurance is provided at no cost to the employee with a plan equal to one year's salary. Amounts over $50,000 will be considered taxable to the employee. Coverage begins on the first of the month following thirty days of employment.
- Disability Insurance is provided at no cost to the employee. Long term coverage commences on the first day of the month following one year of employment.
- Pension Plan - All full-time employees who have attained the age of twenty-one (21) are eligible to participate in the Hamden Surgery Center 401 (k) Profit Sharing Plan following the completion of ninety (90) days of service. Matching contributions to this plan are made by the employer on the first 3% of the employee's salary. Employees will become fully vested after three (3) years or more of service, 75% vested after two (2) years of service and 50% vested after one (1) year of service.
- Vacation & Personal Time - All full-time employees shall begin accruing, on a monthly basis, one hundred and sixty (160) hours of paid vacation/personal time off annually. All vacation/personal time off accrued must be used within one year after it is earned, and cannot be carried into the next year.
- Sick Time - All full-time employees will accrue 64 hours of sick time annually. During the first year of employment the total sick hours will be pro-rated, based upon the number of full months the employee has worked. At the completion of each year, the employee may elect to be paid a cash bonus for any accumulated sick hours in excess of 32. This bonus will be paid on the first pay day in January, at the employee's current rate of pay . The first 32 unused sick hours will be placed in a "bank" and will accumulate from year to year for use at a later time if the need arises. An employee may elect to additionally "bank" any unused sick hours instead of receiving the cash bonus. The hours accumulated in the "bank" will not be included in the calculation for bonus hours paid at the end of the second or subsequent years, or upon termination of employment. Repeated misuse of sick time will be considered an abuse and will be dealt with appropriate discipline, up to and including discharge.
- Holiday Time - the Center will be closed on the following holidays: New Year's Day, Memorial Day, Independence Day, Labor Day, Thanksgiving Day and Christmas Day. Employees will receive 8 hours of holiday pay for those days.
- Tuition Reimbursement - full-time employees will be entitled to tuition reimbursement of $500.00 per calendar year, provide the course is job related and approved by their immediate supervisor. Employees must achieve a 'B' average or better and will be reimbursed upon submission of documentation of course completion.
